To participate in the Junior Sailing Classes this summer, three sets of forms
must be submitted. The registration form, the medical history form, and
the release form. You have two options as to how to submit the forms. You can
click on the links for the printable version of the forms and print them on
your local printer, complete and then mail the forms back in. Another option is
to complete electronic versions of the forms online and submit them via e-mail.
Keep in mind that electronic versions will still require signatures before
classes begin.
In order to view and print the printable versions of the forms, you will need
the freeware Adobe Acrobat Reader on your system. If you do not have that
software, then you can obtain it from the web by clicking on the
Get Acrobat Reader button below:
